Notice is hereby given of the loss of one (!) pad of unused BIR Form No. 0423 Apprehension Slip, bearing the following serial numbers:

APS200300020351-APS200300020400

Cesar Padilla. Revenue District Office No. 80. Revenue Region No. 13, Cebu City but The abovementioned forms were issued to Assistant Revenue District Officer

was reported by Ms. Hilda Praxedes S. Tecson. Administrative Officer V. Revenue District Office No. 80. Revenue Region No. 13. Cebu City due to former's death and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID.
