LOCAL GAMING AGENT REQUIREMENTS (NEW APPLICATION) 1. Letter of intent addressed to Chairman and Chief Executive Officer of PAGCOR; 2. Duly accomplished and notarized application form (OGLD Form No.1052); 3. Duly notarized Agency Agreement; 4. *Securities and Exchange Commission (SEC) Registration; 5. *Articles of Incorporation (AI) and By-Laws duly stamped and received by SEC; 6. *Latest General Information Sheet (GIS) with beneficial ownership declaration page; 7. Company Profile; 8. Organizational Chart indicating person occupying the position; 9. Duly accomplished and notarized Personal Disclosure Sheet (PDS) (OGLD Form No. 1050) of authorized representative and key officers of the corporation; 10. Board Resolution or Secretary’s Certificate appointing Authorized Representative; 11. Bank Certificate proving financial capacity of at least Twenty Million Pesos (Php 20,000,000.00); 12. *BIR Certificate of Registration; 13. Result of evaluation issued by an accredited probity checker of OGLD; 14. Regulatory inspection by OGLD for verification of Philippine office address; and 15. Proof of payment of the Application Fee *Certified True Copy of document NOTE: All documentary requirements must be labeled and arranged in order as listed in the Checklist of Requirements. All documentary requirements must be printed on an A4-size paper for submission to OGLD Office. APPLICATIONS WITH INCOMPLETE INFORMATION / REQUIREMENTS WILL NOT BE ACCEPTED.
