REMOTE OPERATIONS AND ANCILLARY SERVICES DEPT. CHECKLIST OF REQUIREMENTS Page 1 of 3 (DATA STREAMING PROVIDER) A. NEW APPLICATION 1. Letter of Intent addressed to the Chairman and CEO of PAGCOR 2. Proof of Payment of the Application Fee. 3. Duly accomplished and notarized Application Form 4. Certified true copy of the Certificate of Registration from the Securities and Exchange Commission 5. Certified true copy of the Articles of Incorporation (AOI) and By-Laws; i. Authorized Capital Stock of at least Ten Million Pesos (Php10,000,000.00), with a Paid-up Capital Stock of at least Two Million Pesos (Php2,000,000.00); 6. Certified true copy of the latest General Information Sheet including the beneficial owner page 7. Company Profile 8. Organizational Chart indicating name of person and position in the company with signature of the personnel who prepared the same 9. Business Plan / Business Concept including necessary documents relative to the applicant’s operations such as Manuals / Guidelines 10. Duly accomplished and notarized Personal Disclosure Sheet (PDS) of all Directors, Officers and the Authorized Representative of the corporation Note: Form may be downloaded in the ROASD website 11. Board Resolution appointing the company’s Authorized Representative to PAGCOR 12. Copy of the Contract of Lease for the operating site 13. Valid Mayor’s Permit / Business Permit 14. Certified true copy of Bureau of Internal Revenue (BIR) Certificate of Registration 15. Apostilled/consularized copy of the Service Agreement with the licensed gaming operator abroad 16. Apostilled/consularized copy of the company registration documents of the gaming operator abroad (including Register of Directors or foreign equivalent) 17. Apostilled/consularized copy of the gaming operator’s license 18. Notarized Undertaking that the company will not: (i) Accept bets and handle payouts; (ii) Engage in illegal activities; and (iii) Provide services to unlicensed gaming operators 19. Probity Checker Final Report Note: Probity results shall be submitted by the probity checker directly to PAGCOR; the company must ensure timely compliance with the probity requirements to prevent delay in processing. 20. Permit to Possess Gaming Paraphernalia (if applicable) Note: Required only for companies providing streaming services - Must be applied for separately with PAGCOR (co-terminus with the accreditation validity) - Please refer to Page 3 of 3 for the complete list of requirements

Updated Organizational Chart indicating name of person and position in the company 6. Updated Business Plan/Business Concept of the applicant including the necessary documents relative to the operations such as Manuals/Guidelines 7.
