REPUBLIKA NG PILIPINAS KAGAWARAN NG PANANALAPI KAWANIHAN NG RENTAS INTERNAS Quezon City July 28, 2005 REVENUE MEMORANDUM CIRCULAR No. 34-2005 SUBJECT : Missing three (3) original copies of unissued BIR Form No. 17.58- Authority to Cancel Assessment TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing original copies from three (3) sets of unissued BIR Form No. 17.58- Authority to Cancel Assessment (ATCA) with the following serial numbers: ACA97-000015620 ACA97-000015639 ACA97-000015643 The abovementioned copies were reported as missing by Ms. Anita G. Jacinto, Revenue Administrative Officer III, Revenue Region No. 05, Valenzuela City. The entire three sets of said unissued ATCA have consequently been cancelled. All official transactions involving the use thereof are therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
