Additional guidelines in accomplishing Schedule 5 (Other Current Account Transfers Acquisition and Disposition) of the FX Form 1
CIRCULAR LETTER Series of 2003
TO: All Authorized Agent Banks and their Subsidiary/Affiliate Foreign Exchange (FOREX) Corporation
Effective May 12, 2003, the following additional guidelines shall be observed in accomplishing Schedule 5 (Other Current Accounts and Transfers Acquisition and Disposition) of the Foreign Exchange Form 1 Report (FX Form 1) required under BSP Circular No. 194 dated March 30, 1999 and the Weekly Report on the FX transactions of FOREX Corporations required under BSP Circular Letter dated June 27, 2001:
1. Full name of Counterparty shall be encoded. 2. For individuals, the following format shall be observed: SURNAME, FIRST NAME M.I.
