REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE Quezon City June 28, 2012 REVENUE MEMORANDUM CIRCULAR No. 30-2012_ SUBJECT : Loss of several accountable forms TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of the accountable forms hereunder listed together with other pertinent information, to wit: Accountable Form Serial Number Missing Copy/Set BIR Form No. 2524 ROR201003911562- 39 duplicate to quintuplicate copies (Revenue Official Receipt) ROR201003911600 (issued) Accountable Form No. 51 ACF201000018901- 50 duplicate and triplicate copies (issued) (Official Receipt) ACF201000018950 The loss of said accountable forms was reported by Mr. Lloyd P. Amboayan, Revenue Collection Officer, Revenue District Office No. 101, Revenue Region No. 16, Cagayan de Oro City. Consequently, all official transactions involving the use of the lost accountable forms should be verified. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid TCC is found and to undertake necessary measures or actions in order to prevent the improper or fraudulent disposition/use of the same.
