3. Sports Betting System (SB) Service Provider Registration Form -- Application for Accreditation (GS Form No. 52). Provider; and

Duly accomplished Registration Form together with the prescribed documentary requirements shall be submitted to GLDD or emailed the clear scans to [email protected].
