Page No. Page 1 of 2 Form No. GLDD - 993 POKER TOURNAMENT FORM Revision 6 No. Effectivity Aug. 20, 2025 CRM FORM NO. 51 Use this form to request approval from or notify PAGCOR regarding the conduct of poker tournaments, to wit: a) For tournaments with One Million Pesos (Php1,000,000) guaranteed prize pool and above, the Licensee shall request approval from PAGCOR at least fourteen (14) business days prior to implementation date. b) For tournaments with BELOW One Million Pesos (Php1,000,000) guaranteed prize pool, the Licensee only needs to notify PAGCOR of the intended poker tournament at least three (3) business days prior to implementation date. All requests for approval with incomplete information and/or attachment/s shall not be processed. PLEASE NOTE THAT PAGCOR RESERVES THE RIGHT TO CONDUCT POST-AUDIT OF THE POKER TOURNAMENT. For below Php1,000,000 tournaments, fill up Item B of Section A. SECTION A: POKER TOURNAMENT DETAILS (TO BE PROVIDED BY LICENSEE) A. Request for approval of proposed poker tournament with Php1,000,000 and above guaranteed prize pool, with accompanying tournament details attached as Annex ___ B. Notification of poker tournament with below Php1,000,000 guaranteed prize pool, with accompanying tournament details attached as Annex ___ We certify and confirm that: 1. That, the attached tournament details of poker tournament are consistent with PAGCOR’s prescribed standard format and contain the following: a) Title i) Add-on amount (if applicable) b) Format Add-on Chips c) Date and Time Add-on Period d) Prize Pool j) Blind Level Duration e) Buy-in amount k) Blind Structure f) Entry Fee l) Payout Structure g) Starting Stack/Chips m) Calendar of Events, if applicable h) Re-buy amount (if applicable) n) Location of tournament Re-buy Chips o) Service Charge (if applicable) Re-buy Period 2. That, the above tournament details are published/posted in the poker gaming area for the players’ proper information and guidance. 3. That, the PAGCOR Monitoring Team (PMT) will be notified in the event of any cancellation of tournament which will include reason(s) and/or circumstances surrounding such cancellation. 4. That, the PMT will be given a copy of the list of all tournament participants and winners. 5. If submission of this form is done through the ECMS, an e-mail notification confirming receipt shall be received. The status of the application can be viewed and monitored through the online account created using the same system. Operator can also download and print the acknowledgment generated as proof of submission. If no e-email notification is received, the Operator may inquire from the Gaming Licensing & Development Department through the contact details: Tel. The Licensee shall not directly poach on another licensees’ or PAGCOR’s customers by actively enticing or transporting them to the Licensee’s casino. 3.
