REPUBLIC OF THE PHILIPPINES DEPARTMENT OF FINANCE BUREAU OF INTERNAL REVENUE 25 April, 2003 REVENUE MEMORANDUM CIRCULAR No. 25-2003 SUBJECT : Loss of Four (4) sets of unused BIR Form No. 0421 � Tax Verification Notice TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of four (4) sets of unused Tax Verification Notice (BIR Form No. 0421) with the following serial number: TVN200100007628 TVN200100007632 TVN200100007630 TVN200100007634 The abovementioned forms were reported as lost by MS. ADELINA L. TAN, of BIR-Revenue District Office No. 112, Revenue Region No. 19, Davao City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
