TO : All Regional Directors, Revenue District Officers, Employees and Others Concerned ________________________________________________________________________ Notice is hereby given for the loss of one (1) original copy of Tax Credit Certificate (TCC) to wit: TCC No./Serial No. Date of Issue TCC Face Amount TCC Balance P 6,125,213.09 P 3,000,000.00 R-018104 August 20, 2008 The abovementioned TCC was reported as lost on February 13, 2012 by Mr. Alberto M. de Larrazabal, Chief Financial Officer of the subject taxpayer, and indorsement dated February 16, 2012 of Atty. Villamor Ventura S. Plan, Executive Director, DOF-OSS-Center. Consequently, the entire set bearing the said TCC No. is hereby cancelled. All official transactions involving the use of the said form effective February 14, 2012 is therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid TCC is found and to undertake necessary measures or actions in order to prevent the improper or fraudulent disposition/ use of the same.
