Page No. Page 2 of 3 ELECTRONIC GAMING MACHINE Form No. GLDD - 999 SHUTDOWN NOTIFICATION FORM Revision 6 No. Effectivity Aug. 20, 2025 We confirm and undertake to do the following: ▪ A manual meter reading shall be conducted by the Licensee together with the PMT before EGM shutdown. ▪ A functionality test shall be conducted on the EGMs in order to ensure that actual machine settings will reflect the approved parameter settings and EGM transactions are captured by the Casino Management System. Functionality test shall be conducted based on a checklist which shall be prepared by the Licensee. The results of the functionality test should be favorable prior to commercial operation of the EGMs. ▪ In the event that the EGM shutdown is emergency in nature, machine meter values prior to shutdown shall be derived from the casino management system. Once power has been restored, manual meter reading shall be conducted. Thereafter, a functionality test shall also be conducted to ensure that the EGM’s current machine configurations and EGM transactions are captured by the casino management system. Said functionality test shall be based on a checklist which shall be prepared by the Licensee. Functionality test results should be favorable prior to commercial operation of the EGMs. ▪ The abovementioned undertaking shall be carried out in close coordination with the Compliance Monitoring and Enforcement Department – Integrated Resorts and the PMT. Hard copies may be submitted directly to the PMT office. If submission of this notification form is done thru e-mail, the Licensee should ensure that the email has been duly received and acknowledged by PMT. The Licensee shall be notified by PMT also thru e-mail confirming receipt of this form. If no e-mail confirmation is received from PMT, the Licensee should submit the hard copy directly to PMT.
