ANNEX A

GUIDELINES IN THE ACCOMPLISHMENT OF THE REPORTING MATRIX ON

SHIPBOARD PROVISIONS AND VESSEL STATUS

All Licensed Manning Agencies (LMAs) are hereby guided to ensure the proper and accurate accomplishment of the prescribed reporting matrix. Each row in the matrix shall correspond to one vessel and must contain complete, current, and verified information, in coordination with the concerned shipowner, principal, and vessel master. No field shall be left blank; in instances where information is not immediately available, the notation "For verification" may be indicated, provided that updates are subseguently submitted. All entries shall follow consistent units of measurement, particularly in terms of days, percentages, and headcount.

The basic vessel information shall include the full name of the LMA, the registered vessel

as far as practicable, be indicated using latitude and longitude coordinates; otherwise, the name, and the correct seven-digit IMO number. The current location of the vessel shall

nearest port or general sea area may be specified. The next intended port of call shall likewise be declared. The number of Filipino crew members onboard and the total number of crew, regardless of nationality, shall be accurately reflected.

With respect to shipboard provisions, all LMAs shall report on the status of essential supplies, including food, potable water, fuel, medical supplies, and other essential consumables. For each category, the condition shall be classified as "Sufficient, "Adequate," or "Critical," based on actual onboard inventory, consumption rate, and

condition of supplies. operational requirements, with brief remarks provided where necessary to clarify the

For uniformity in reporting, the following standard definitions shall be observed. For food and potable water, a supply level exceeding Thirty Days (30) days shall be considered "Sufficient," indicating that normal operations may be sustained without immediate concern. A remaining supply of Fifteen (15) to Thirty (30) days shall be considered

replenishment. A remaining supply of less than Fifteen (15) days shall be considered "Adequate," indicating the need for close monitoring and advance planning for "Critical," reguiring immediate action to prevent risk to the health and welfare of the crew.

In terms of fuel or bunker levels, a remaining capacity of more than fifty percent (50% shall be classified as Sufficient, while levels between twenty percent (20%) and fifty percent (50%) shall be considered "Adequate," necessitating close monitoring and scheduling of refueling operations. Fuel levels below twenty percent (20%) shall be deemed "Critical" and shall require urgent replenishment to ensure the safe and continuous operation of the vessel.

For medical supplies and other essential consumables, the classification shall be based on availability and criticality of items. Supplies that are complete and within required

immediately affect safety or health shall be classified as "Adequate. However, the operational levels shall be considered "Sufficient." Minor shortages that do not

absence or insufficiency of essential, life-saving, or mission critical items shall be classified as "Critical" and shall necessitate immediate corrective measures.

LMAs shall likewise indicate the estimated number of days remaining for both food and potable water based on actual onboard consumption rates, and not merely on rough estimates. The fuel status shall be expressed in percentage terms.

The column on "Immediate Assistance Needed" shall clearly state whether support is required, and if so, shall specify the exact nature of assistance needed, such as urgent resupply of provisions, fuel replenishment, medical assistance, or other necessary interventions

The "Action Taken by Company" shall reflect concrete and verifiable measures already undertaken by the LMA, shipowner, or principal, including coordination with port agents or ship chandlers, arrangements for replenishment at the next port, implementation of conservation measures, or escalation of concerns to relevant authorities.

All entries in the reporting matrix shall be clear, specific, and measurable. General or vague descriptions such as "normal" or "okay" shall not be accepted. The accomplished matrix shall serve as a critical basis for monitoring, assessment, and possible intervention by the Department of Migrant Workers, and as such, utmost diligence and accuracy in reporting are hereby enjoined.
